HPNAP
APPLICATIONS
The
2012-2013 HPNAP applications were
mailed to all eligible agencies on April 27. They are now available on our
website (Word or Adobe format). Original documents must be returned to Long
Island Cares. Do not fax or email applications back. Mail the applications
back by May 31, 2012. Faxed or emailed applications will not be accepted.
All agencies must fill out the Food Application. If you need Operation
Support (OS) and/or Capital Equipment (CE), please fill that grant
application as well. MAKE SURE ALL QUESTIONS HAVE BEEN ANSWERED!

Monthly Reports
Every member agency must submit a Monthly Report. We
are required by government agencies to obtain this information. Even
when you are closed, renovating, relocating, or have no food to give out,
you must submit a Monthly Report by the 10th of each
month. We need this information to
provide service to you. Please send your report
mail, fax, phone, or e-mail.
